Superbills

If I am not in network with your insurance company, I can offer a superbill after your appointment upon request. You may submit this to your insurance company for possible reimbursement.

Online Payments

Payment is due at the time of service. Most major credit cards, HSA, and FSA are acceptable forms of payment. All clients are required to have a credit card on file to reserve appointments. 

Please note: A minimum of 48 hours notice is required for appointment cancellations or rescheduling. In the event of a no-show or a late cancellation, you will be charged the full appointment fee. To secure your appointment slot, a valid credit card on file is required.

What Questions Should I Ask When Calling My Insurance Company?

 

Since payment for services is ultimately your responsibility, I recommend calling the number on the back of your insurance card and asking them the questions below.

 

    • Does my plan cover nutrition counseling, procedure codes 97802 and 97803?  If they say you don’t have coverage with those codes, ask them about CPT codes: 99401, 99402, 99403, and 99404.
    • Is nutrition counseling covered when via telehealth? Note: I only offer telehealth services at this time
    • Is nutrition counseling covered as a preventive benefit?
      If so, are there certain criteria that must be met to qualify (e.g. “obesity” or cardiovascular risk factors)? 
    • Is nutrition counseling covered as a medical benefit?
      If so, are there any excluded diagnoses?
      • If the insurance asks for a diagnosis code, ask about diagnosis code Z71.3 or Z72.4.  
      • If you have an eating disorder, ask the insurance about your specific eating disorder diagnosis in addition to the Z71.3 and Z72.4
        • Anorexia, restricting type: F50.01
        • Anorexia, binge eating/purging type: F50.02
        • Bulimia: F50.2
        • OSFED (atypical anorexia or bulimia): F50.89
        • ARFID (avoidant/restrictive food intake disorder): F50.82
        • Binge eating disorder: F50.81
    • Do I need to meet my deductible before insurance will cover outpatient nutrition services? 
    • Do I have a co-pay for nutrition counseling?
    • Do I need a referral or prior-authorization?
    • How many visits do I have per calendar year? 
        • Depending on the insurance plan, the number of visits may vary from 0 to unlimited
